Most people who contact us about drug rehab in Spain are not looking for Spain. They are looking for distance. Distance from the people they use with, from the streets where it happens, and from a life that has quietly organised itself around a substance. Spain is where we happen to be, and it turns out to be a good place to do this work.
InHarmoni is a private residential clinic in El Paraíso, minutes from the beach and a short drive from Marbella. We take no more than ten clients at a time, we work in English, and every programme is overseen by qualified medical and psychotherapeutic staff. The clinic is licensed in Spain as a healthcare provider.
What we treat
These are the substances we work with most often:
- Cocaine and crack cocaine
- Ketamine
- Codeine, tramadol and other prescription painkillers
- Benzodiazepines and prescribed sleep medication
- Alcohol, whether on its own or alongside drug use
We are also clear about our limits. Dependence on street heroin generally needs longer term treatment than a 28 day residential programme can provide, and we do not present ourselves as the right option for it. Where someone contacts us in that situation we assess it individually and say so.
Why treatment away from home works
Drug use is bound up with place, routine and the people around you. Trying to stop while facing every cue every day is the reason most attempts fail in the first fortnight. A month somewhere else breaks that chain in a way a month at home cannot.
There is a second reason, and clients raise it more often than the first. Treatment abroad is discreet. For someone whose professional life would be complicated by a local admission, that is not a luxury, it is what makes asking for help possible at all.
Medically supervised withdrawal
How dangerous withdrawal is depends entirely on the substance. Alcohol and benzodiazepines can produce seizures and need a carefully managed reduction. Stimulant withdrawal is rarely dangerous but the crash is punishing, and it is where unsupported attempts collapse. Opioid withdrawal is physically severe.
Medical detox is included in the 28 day programme, with monitoring around the clock, medication where it is appropriate and a doctor reachable at any hour of the night. We assess which of these applies to you before you travel, not after you arrive.
How the programme is built
Assessment. A clinical conversation by phone or video before arrival, covering substance use, physical health, medication, mental health and what has been tried before. This is also where we are honest about whether our programme is the right fit.
Medically supervised withdrawal. Managed with medical oversight, so that the first week is safe and considerably more bearable than most people expect.
Therapy. The part that decides whether recovery holds. Individual sessions, group work, cognitive behavioural therapy, schema therapy, acceptance and commitment therapy, trauma informed work where relevant, and relapse prevention built around the specific situations that trigger use.
Aftercare. Every 28 day programme includes four months of individual aftercare with a case manager. The weeks after treatment carry the highest risk, and this is the part that is usually missing when someone relapses after a stay somewhere else.
Coming from the United Kingdom
Most of our clients travel to us. Malaga airport is under an hour from the clinic, with direct flights from most British and Nordic cities. You need a valid passport and travel insurance. The assessment happens before you fly, so nothing is uncertain on arrival, and we arrange transfers in both directions.
The setting
Ten private rooms with en suite bathrooms, 2 500 square metres of garden, a heated pool and quiet places to sit. It is deliberately small and it is not designed to feel like a hospital. Clients describe the first real sleep in months within the opening week, and that is usually where the work starts.

Frequently asked questions
How long does drug rehab in Spain take?
The residential programme is 28 days as standard. There is a shorter 14 day option and a longer 56 day one, and which is right depends on the substance, the length of use and what else is going on. We say honestly at assessment if we think you need longer.
Is medical detox included?
Yes, in the 28 day programme. Detox is not included in the 14 day programme, which is one of the reasons we ask about substance and dose before recommending a length.
Which drugs do you treat?
Cocaine and crack, ketamine, prescription opioids including codeine and tramadol, benzodiazepines, prescribed sleep medication, cannabis and alcohol. Street heroin sits outside what a 28 day programme can do properly.
Is treatment confidential?
Entirely. Clients are not identified, and nothing about your stay is shared with an employer, insurer or family member without your written consent.
Do you treat mental health conditions at the same time?
Yes, and we usually have to. Depression, anxiety, trauma and burnout sit underneath addiction more often than not. Treating an addiction and a mental health condition together is known as dual diagnosis, and it is most of what we do, because treating the substance while leaving the reason for it untouched is the most reliable route back to where you started.
What happens after I leave?
Four months of individual aftercare with a case manager, agreed before you travel home, covering continued therapy, contact with us and the practical changes that stop the old routine from reasserting itself.
